At NorthBay Health, we are seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team in Vacaville, CA. In this role, you will serve as a key member of a multidisciplinary care team, delivering high-quality, patient-centered care across the lifespan from pediatric to geriatric populations while working collaboratively with a supervising physician and guided by established clinical protocols.

- Graduate of an accredited Nurse Practitioner (Master s required) or Physician Assistant program
- Current California NP or PA license
- Board Certification (NCCPA for PA or ANCC, AANP, AACN, or PNCB for NP)
- Current AHA (or equivalent) CPR certification
- Active DEA license (Schedule II and above)
- Valid Furnishing Number
- NP: 2+ years of NP experience or 4 years of RN experience
- PA: Relevant clinical experience or completion of internship
- Preferred: Experience in primary care or ambulatory setting
- Ability to evaluate and treat patients across the lifespan (pediatric to geriatric)
- Strong collaboration, communication, and patient care documentation skills
