Need: PA/FNP

When: 12/1/22 - 2/23/23

Day: Thursday's only

Shift: 8am to 4:30pm with an unpaid 30-min lunch

Cambridge City , IN: NP only

When: Dec 16, 27, 28, 30

Shifts: Mondays 8 am - 5pm Tuesdays 8 am - 4pm Wednesdays 8am - 6pm Fridays 7am - 4pm Closed from 12pm-1pm daily for lunch

Duties: Primary care and chronic disease management. Some light occupational health, DOT physicals

Whiting, IN: NP or PA OccMed

When: ASAP to Nov 30

Shift: 5am to 1:30 pm with an unpaid 30-minute lunch

Duties: Occ Services (med surveillance, annual physicals, DOT drug testing, case management, RTW services)

Clinic is open from 5 am to 1:30 pm daily and the surveillance exams are done between 5-7am. Additional Staff includes RN, 2 MA's and 1 front desk

REQUIREMENTS:

IN State License

DOT certification

BLS

At least one year of experience
