Requirements

• Board Certified
• Active IA License or Active IMLC LOQ
• Must be able to cover most or all of the specified dates
• Fellowship Trained (General Radiology with Fluoroscopy)
• Clean Malpractice/No Background
• Must be comfortable doing Fluoroscopy
• Self-Query NPDB Report pulled within 30 days at time of presentation
